Customizations > Forms lets you change the questions on the forms in your directory, such as the contact form on partner profiles, the partner application and the Get Listed sign-up. You can add your own fields, reword or remove optional ones, and show a field only when another answer matches. This article is for vendors.
Who can do this: Admin role (Managers can view forms) · Availability: available on request: contact PartnerPage support to turn it on. Each form appears when its feature is on for your directory.
Forms you can customize
The Forms list shows each form your directory uses, with a Customized column that says whether you changed it. The following table lists the forms.
Form | Who fills it in | Where | Listed when |
Contact request | Visitors | The contact page of each partner profile | Contact requests are on |
Matchmaking request | Visitors | Your directory's matchmaking page | Matchmaking is on |
Get listed | Partners signing up | Your Get Listed form | Always |
Partner questionnaire | Partners | Their profile editor; only your team sees the answers | The partner questionnaire is on |
Partner application | Companies applying to your program | Your directory's apply page | Partner applications are on (on by default) |
Deal registration | Partners | Register a deal in their dashboard, and the public deal registration link | Deal registrations are on |
Post-call feedback (partner) and Post-call feedback (customer) | The partner and the customer | After a call booked with the Scheduler | The Scheduler with post-call feedback is on |
Customize a form
Customizing a form starts from PartnerPage's default fields, so you only change what you need. To customize a form:
In your directory's sidebar, click Customizations, then Forms, then the form's name.
Turn on Customize. The default fields become editable. (The Partner questionnaire has no default form and no Customize toggle: you add its fields directly.)
Click Add new field, choose a field type, fill in its settings and click Save in the field window.
Drag fields to change their order. Click Edit to change a field, or remove it.
Try the form in the preview beside the fields. Submit in the preview checks required fields and rules but sends nothing.
Click Save.
You see "Form saved", and the form uses your fields from then on. Some core fields of the default form can't be edited or removed; they have no Edit button.
Field types
Add new field offers these field types, grouped as in the menu:
Group | Field types |
Text | Single line text, Multi-line text, Email, URL |
Numeric | Integer, Decimal, Money, NPS |
Selection | Single choice, Multiple choice |
Other | Date, Checkbox, Consent, Custom content, and on forms whose default has them, Phone and Comments when they aren't already on the form |
Most fields have a Field label and a Required option. Depending on the type, you also set a Placeholder, minimum and maximum characters or values, Decimal places, a Currency code, or the Options of a choice field. Choice fields can add an "Other" option where people type their own answer, and Multiple choice can set how many options people must pick.
Consent and Custom content fields
Consent and Custom content fields hold text with links, written in HTML:
Consent is a checkbox with your wording in Label (HTML), up to 1,000 characters, for example "I agree to the terms and conditions" with a link. Mark it Required if people must tick it. Use a Consent field to link your privacy policy: the Privacy policy and Terms of service links in Configuration don't appear on your forms.
Custom content is display-only text in Custom content (HTML), up to 1,000 characters, such as a note about how you use the data. It collects no answer.
Both accept these HTML tags: a, p, div, span, strong, em and br. Links can start with https://, http://, mailto: or tel:.
Internal label
The Internal label of a form field is the field's fixed name in the PartnerPage API and in webhook payloads, so your integrations keep working when you reword the field label. An internal label is optional, can use letters, numbers, spaces, hyphens and underscores, must be unique within the form, and can be up to 255 characters. Fields from the default form have fixed internal labels that start with partnerpage-.
Changing an internal label changes API responses and webhook payloads for that field and may break existing integrations. CSV exports name each custom field's column after its Field label, not its internal label.
Show a field only when another answer matches
Conditional visibility in a field's settings shows or hides the field based on answers to a Single choice or Multiple choice field. To add a condition:
Open the field's settings and choose Show when or Hide when.
In the condition row, choose the field (Select field), the operator (is or is not) and the value (Select value).
To combine conditions, click Add condition and choose whether Conditions match with AND or OR.
A field that other fields depend on can't be removed: you see "Cannot remove field" until you change those conditions.
Restore the default form
Turning off Customize puts back PartnerPage's default form. To restore the default form:
Open the form and turn off Customize.
Click Save, then Restore default form in the "Restore default form?" dialog.
Your custom fields are removed. Answers already submitted through them stay saved, but you'd need to recreate the fields if you customize the form again.
Change the partner questionnaire
Saving changes to a Partner questionnaire that already has fields asks you to confirm, with the warning "Previous answers of the form will be lost and partners will have to fill it again." Plan your questions before partners start answering.
Where custom answers appear
Answers to your custom fields appear in the CSV exports of Contact requests, Matchmaking requests and Partner applications, one column per field named after its label. Consent answers and Custom content aren't exported as custom columns. See Export partners, applications and reviews.
